INFLUENCE OF ACUPUNCTURE ON DOPPLER ULTRASOUND IN PREGNANT WOMEN.

OBJECTIVE: To evaluate the influence of acupuncture on the blood flow in the umbilical artery (UA), fetal aorta and uterine artery and on the fetal heart rate using two different acupuncture points (SP-6 (Sanyinjiao) and LI-4 (Hegu)). METHODS: In group A (n = 50), measurements (Doppler ultrasound and fetal heart rate) were performed at term after an uneventful pregnancy (#1) followed by acupuncture treatment using the acupuncture-point SP-6 bilaterally. The treatment time lasted 15 min after which the next measurement (#2) was carried out. The needles were then inserted into the LI-4 acupuncture point for a further 15 min. A third measurement at the end of acupuncture treatment (#3) completed the session. In group B (n = 25), measurements were made before (#1) and after (#4) acupuncture at LI-4 acupuncture points only. RESULTS: In group A, the mean systolic/diastolic (S/D) ratios of UA #1, UA #2 and UA #3 were 2.45, 2.38 and 2.22, respectively (P = 0.0012). The difference in mean S/D ratios between UA #1 and UA #3 as well as that between UA #2 and UA#3 were statistically significant (P = 0.0002 and P = 0.008, respectively). There was no difference between the mean S/D ratios of the uterine artery and between the mean resistance indices of the fetal aorta. In group B, the only significant difference between measurements following acupuncture treatment was in fetal heart rate (139 vs. 143 bpm, P = 0.02). CONCLUSION: Our study indicates a positive influence of acupuncture treatment on umbilical artery waveforms when using a combination of SP-6 (Sanyinjiao) and LI-4 (Hegu) acupuncture points. Acupuncture performed at these sites either individually or in combination does not seem to affect blood flow in the fetal aorta or uterine artery.

In this paper, 92 cases of obstetrical department disease were chosen for studying,and were randomly divided into examining group and calculating group. Among them, there are 57 cases with spleen-qi deficiency and 35 cases without it. A multiple regression analysis on 47 kinds diagnostic factors which were related to the type of spleen-qi deficiency was conducted and a diagnostic equation was obtained after the retrospective and prospective studying on it. It was also found that the main diagnostic factors which were chosen could be divided into two types. One was general diagnostic factors such as inappetence, emaciation, bodily indolence, sallow complexion and so on the other was specific diagnostic factors such as vaginal bleeding and edema during pregnancy, amniotic fluid, galactorrhea etc. The practical TCM obstetrical department disease with spleen-qi deficiency syndrome could be made up of the obove two types. The quantification diagnostic standard was preliminarily set up according to the regression equation as well.

Based on the medical property of Semen Vaccaria, a prospective comparative study was carried out between the antenatal auricular Vaccaria-pressing stimulation test (AVPST) and the traditional non-stress test (NST) in 152 normal term pregnancies. The results showed that the positive predictive values and the specificities of both test were all very high. The negative predictive values, sensitivity and total effective rate of AVPST were higher significantly than those of NST, P < 0.01, but the false negative rate of the former was lower than that of the latter, P < 0. 01. These results revealed that due to its low negative rate and false negative rate AVPST is of great clinical value in antenatal fetal monitoring. By AVPST the predictive value of negative result was elevated. AVPST is meaningful in fetal monitoring, that is, positive result of AVPST indicates favourable condition of fetus, while negative result means unfavourable prognosis.

The author has studied the materials on menstruation, gestational course, embryonic development and hygiene during pregnancy recorded in ancient Tibetan medical literatures, and compared them with that in Han medicine, generally known as TCM. The article points out that Tibetan and Han medicine share some common views in pregnancy and embryology, but, in the aspects of observation and knowledge in menstruation and embryology, Tibetan medicine seems more unique. Tibetan medicine not only recorded foetal shape week by week for 38 weeks, but also presented them in color pictures to illustrate the "fish-tortoise-pig" stages in its course of embryonic development which is absent in Han medicine. Tibetan medicine is very creative in its knowledge on pregnant physiology and embryology.

In order to assess the clinical availability of early pregnancy detection with Sphygmogram of smooth pulse, pulse pictures and endometrial tissues were simultaneously examined respectively with sphygmograph and pathological method in 205 cases of clinically diagnosed early pregnancy. Among them, 196 cases were pathologically confirmed as early pregnancy, 192 displayed smooth pulse on the Shygmogram. This shows the precision of sphygmogram of smooth pulse in detecting early pregnancy was 97.6%, its sensitivity 97.9%, Jordan index 0.868, specificity 88.9%, and misdiagnosis 11.1%. Though its specificity was not quite satisfactory, sphygmogram of smooth pulse has proved to be a sensitive index with high precision and easy management. Therefore, it is well worth to be popularized in China.

The myoelectrical activity of the uterus is observed by implantation of bipolar electrodes in uterine smooth muscle of fermal rabbits. The effect of electroacupuncture at Sanyinjiao (SP-6) and Cuiqing on uterus is studied in this article as the indicators of the myoelectrical activity. The mechanism of electroacupuncture is investigated by electroacupuncture at Sanyinjiao and Cuiqing points after sympathectomy. The result show as follows : electroacupuncture increases significantly both frequency and amplitude of the myoelectrical activity in uterus (P<0.05). This activity disappeared mostly after sympathectomy (P<0.05). The results indicates that the sympathetic nerve may be play an important role in transferring the effect of electroacupuncture to uterus ; but other ways may exist.

The Author focused his attention on the involvment of the physiopatology of Xue and other structures in the determinism of some diseases that often complicate seriously the evolution of a pregnancy. The Author enphathised the common ethiopatogenetic source of different diseases such as gravidarum hyperemesis, hypertension, convulsions, coma, oedema, olguria, proteinuria, depression, diabetes and jaundice of the pregnancy. The interpretation of these disease as complication of an alteration of an unstable state due to: 1- excessive accumulation with stagnation of xue in the Bao and in the low part of the body, 2-weakness of the jing of kidney, 3-weakness of the xue of the Zang, 4-disfunction of Chong and Ren Mai allowing a preventive identification of the patients that may evolve in this complications. So it is possible to try a preventive treatment of this disposition to the decompensation through a dietetic and an acupunturistic treatment. The author proposes some basic pricipales in order to prevention the acute lack of balance of this unstable state characterizing the pregnancy.

Depuis les années 30, la France a pris une place prépondérante dans l'acupuncture en Occident qui connaît un important développement grâce à la traduction des textes fondamentaux de la médecine chinoise, grâce à la collaboration entre les sinologues et les acupuncteurs, et grâce aussi aux expérimentations cliniques qui découlent de ces travaux. Nous voudrions montrer ici trois aspects de cette recherche avec les conséquences pratiques qu'ils impliquent. Nous avons choisi d'une part les conséquences médicales et pratiques qu'apporte la compréhension des viscères comme mode de gouvernement du corps d'après le chapitre 8 de Suwen. Nous avons pris l'étude d'un grand méridien (taiyin) à travers les textes, en montrant comment se dégage une idée commune à ce méridien qui se trouve dans certaines maladies cliniques et comment on les traite. Enfin, nous avons choisi un point d'acupuncture zhongji (3RM) avec ce que nous pensons de son mécanisme physiologique et l'action qu'il peut avoir dans certains troubles comme les menaces d'accouchement prématuré. Chacun de ces aspects de notre recherche est illustré par une observation clinique.Toutes ces recherches essayent de rester conformes à la tradition médicale chinoise et d'en saisir tous les développements cliniques et thérapeutiques.

Les maladies gynécologiques ne peuvent être étudiées sans avoir pris connaissance de la conception chinoise de la femme. Seul le chapitre 1 du Su Wen peut établir le profil de la femme chinoise : "chez la fille à 7 ans, Shen Qi (l'émanation rénale céleste) abonde, la denture change, la chevelure s'allonge (Shao Yin, Rein) ; à 2 x 7 ans = 14 ans la vie sexuelle (Tian Gui) apparaît (puberté). Le vaisseau de la conception (Jen Mo) devient perméable, le Tai Chong (grand assaut) ou Chong Mo s'est pleinement développé, les menstrues amènent régulièrement la possibilité de fécondation. A 3 x 7 ans = 21 ans, l'émanation rénale céleste (Jing Qi) est arrivée à son harmonie maximum avec le développement des dernières dents (dents de sagesse). A 4 x 7 ans = 28 ans, les muscles et les os sont fermes, la chevelure atteint sa plus grande longueur et le corps a sa pleine vigueur. A 5 x 7 ans = 35 ans, le méridien de Yang Ming dépérit, le visage commence à se faner et les cheveux à tomber (les 3 Yang à la tête n'arrivent pas aussi bien). A 6 x 7 ans = 42 ans, les trois méridiens Yang dépérissent dans le haut du corps, toute la face se dessèche et les cheveux blanchissent. A 7 x 7 ans = 49 ans, le Jen Mo est flasque (peu de sang), le Chong Mo = Tai Chong atrophié, le Tian Gui (vie sexuelle) épuisé, les voies souterraines (Tzeu Mo, trajets internes) des règles sont coupées et apparaît la non-fécondité résultante de l'épuisement de Jen Mo, Chong Mo et Yang Ming dans le corps".

Etude chez la femme enceinte : à 3 mois 42 points, à 6 mois 20, à 7 mois 23, à 8 mois 86, à 9 mois 128, la plus forte valeur est obtenue juste au moment de l'accouchement (169). Aprés un bref rappel de la notion d'énergie avec ses différentes lois et théories, l'auteur met en place toute la physiologie énergétique chez la femme telle qu'elle a été établie par la médecine chinoise. Sont décrits, les méridiens intéressant l'appareil génital de la femme, le processus de la grossesse, l' échappement du yin et la lactation. les principales maladies gravidiques ont été taitées avec leur physio-pathologie énergétique, leurs signes cliniques et leurs principes de traitement. Cliniquement, trois observations sur les vomissements incoercibles de la grossesse ont été présentées ; l'acupuncture seule se révèle efficace dans tous les cas. Par contre, sur les six menaces d'accouchement prématuré, l'acupuncture utilisée secondairement aux antispasmodiques classiques a permis l'espacement ou même l'interruption des drogues plus rapidement.
